Please Note:
- We are entirely cash-based via our membership program. Insurance is not required or accepted as a form of payment.
- Entering payment information is required to complete your enrollment form. You will not be billed until your enrollment is completed.
- There is a non-refundable $100 enrollment fee required to be scheduled. Prepaid cards are not accepted as a form of payment.
- In order to receive appointment reminders and to communicate about health matters via email you must provide consent to non-secure forms of communication.
- Be prepared to complete the entirety of the enrollment form, including entering all your health information (medical history, diagnoses, medications, allergies, surgeries, etc). All information must be entered completely prior to being accepted as a patient.
